Idaho Department of Labor

 Idaho Business Registration Form  (IBR-1 Form):
This is the application/registration form used by Idaho employers and businesses that make retail sales to apply for an Idaho withholding account, register unemployment insurance accounts, and apply for sales and use tax accounts. This joint registration form is provided by Idaho Department of Labor, Idaho State Tax Commission and Idaho Industrial Commission. The agencies use the information from the form to determine reporting requirements for various taxes and insurance programs in Idaho, including unemployment insurance and workers' compensation insurance.

An on-line version of Form IBR-1, called Idaho Business Registration System, may be completed and submitted on-line at https://labor.idaho.gov/applications/ibrs/ibr.aspx.. 

Quarterly Payroll Reports
Once registered for unemployment insurance coverage, an employer must make quarterly reports and payments. Reports and payments are due the last day of April, July, October, and January for the quarter ending the previous month. During the last week of each quarter Idaho Department of Labor mails report forms to all known covered employers. The forms can also be found on the following Web sites:

  • TAX020 - Download Form 20 (Employer Quarterly Unemployment Insurance Tax Report) and/or instructions. Available only in Adobe PDF format.
  • TAX026 - Download Form 26 (Unemployment Insurance Wage Report ) and/or instructions. Available only in Adobe PDF format.

New Hire Reporting
All Idaho employers are required to report newly hired employees and those who are rehired after an absence of 12 months or more to Idaho Department of Labor within 20 days of the date of hire. This reporting is part of the national effort to reform welfare. IDOL consolidates employer reports into a statewide directory of new hires, which is then used to locate parents who fail to pay child support.   • W-4  IRS form W-4 is used to report New Hire information. Download the W-4 (Employee's Withholding Allowance) and many other forms from the Internal Revenue Service (IRS) Web site located at http://www.irs.gov. Forms are available in PDF, PCL, or Postscript format. To download the most recent version of W-4, go to http://www.irs.gov/pub/irs-pdf/fw4.pdf?portlet=3.

Farm Labor Contractor Forms: Farm labor contractors must be licensed and bonded.  Forms can be found at http://cl.idaho.gov/pdf/flc-004.pdf. Information concerning the licensing process can be found at http://cl.idaho.gov/pdf/flc-001.pdf.

Several types of forms and related fee schedules are available on-line from the Secretary of State's office. Among them are:

  • Business Entity forms (partnership, corporation, etc.) to register your business entity type
  • Assumed Business Name forms ("Doing Business As" forms) to register your business name
  • Trademark and Service mark forms (for registration in Idaho only)
  • Notary Public forms
  • UCC/Lien forms

   Bureau of Citizenship and Immigration Service

The form most commonly used by employers is the Employment Eligibility Verification Form, I-9. For information about the I-9 visit http://www.uscis.gov/files/nativedocuments/m-274.pdf. To download the form, visit  http://www.uscis.gov/files/form/i-9.pdf

All employees must complete an I-9 during their first day of employment.  Employers have until the end of the third day of employment to complete their portion of the form. The forms are kept in the company's personnel files; they are not mailed. It is extremely important that you have a completed I-9 for every employee.  If an inspector comes to your office and you do not have the forms on file, you could be fined for each missing form.

It is important to note that Sole Proprietors and Partners are required to maintain a completed Form I-9 for themselves in their files, as well as maintaining forms for their employees. For more information about employee issues, visit the Employer Issues section of this Web site.

Internal Revenue Service

Numerous forms are available from the IRS, including tax reporting forms, withholding forms, tax filing forms, self-employment tax forms, and much more. One of the most requested forms is SS-4, Application for Employer Identification Number.

Employer Identification Number: You may need to obtain an Employer Identification Number (EIN), also called a tax identification number, if you have employees or do business with corporations and government agencies. Your bank may also require that you have an EIN before you can open an account, even if you have no employees.  (Social Security numbers are no longer used for business identification purposes.) 

You can find information about form SS-4, Application for Employer Identification Number, on the Internal Revenue Service Web site at http://www.irs.gov/businesses/small/article/0,,id=98350,00.html. You can apply for an EIN by phone, fax, or mail. There is no cost and the process is quick and easy.
