Bureau of Occupational Licenses
The Bureau of Occupational Licenses Web

includes information on the licensing application process, the complaint process,
and access to the necessary forms for each professional board that has contracted with
the Bureau for licensing services.
Not all professional boards contract with BOL, such as the Board of

Idaho
Department of Insurance
The Department licenses insurance agents and third
party insurance producers, as well as certifying fire
marshals and licensing fire sprinkler installers,
fireworks wholesalers, and bail bond agents. The

Idaho Business
Registration Form (IBR-1):
This joint
registration form is provided by Idaho Department of Labor, Idaho State Tax
Commission and Idaho Industrial Commission. Idaho employers and
businesses that make retail sales submit the form to apply for an Idaho withholding
account, register unemployment
insurance accounts, and apply for sales and use tax accounts. The information
from the form is used to
determine reporting requirements for various taxes and insurance programs in Idaho, including
unemployment insurance and workers' compensation insurance.

Quarterly Payroll
Reports
Once registered for unemployment insurance coverage, an employer must make
quarterly reports and payments. During the last week of each quarter Idaho Department of Labor mails report forms to all
known covered employers.
New Hire Reporting
All Idaho employers are required to report
newly hired employees and those who are rehired after an absence of 12
months or more to the Department of Labor within 20 days of the date of hire. This reporting is part of the national
effort to reform welfare. IDOL consolidates
employer reports into a statewide directory of new hires, which is then used to locate parents who fail to pay child

Farm Labor Contractor Forms: Farm labor contractors must be licensed and
bonded. Information about the licensing process and

Farm labor contractors must also comply with federal labor

Several types of forms are available on-line from the Secretary of State's office.
Among them are:

your business entity type

register your business name

It may be legal to use the same name as
another business, depending on your entity type, but it may not be in your best
interests, particularly if the other business has a less
than stellar reputation.

The form most commonly used by employers is the Employment Eligibility Verification

All employees must complete an I-9 during their first day of
employment. Employers have until the end of the third day of
employment to complete their portion of the form. The forms are kept in
the company's personnel files; they are not mailed. It is extremely
important that you have a completed I-9 for every employee. If an inspector comes to your office and
you do not have the forms on file, you could be fined for each missing
form.
It is important to note that Sole Proprietors, Partners,
and some LLCs are required to maintain a completed Form I-9 for
the owners, as well as for their employees. For more

Internal
Revenue Service
Many forms are available from the IRS,
including tax reporting forms, withholding forms, tax filing
forms, self-employment tax forms, and more.
One of the
most requested forms is SS-4, Application for Employer
Identification Number. You may need to obtain an Employer Identification Number (EIN),
also called a tax identification number, if
you have employees or do business with corporations
and government agencies. Your bank may also require you to have an EIN before you can open an account, even if you
have no employees. (Social Security numbers are no
longer used for business identification
purposes.)

You can apply for an EIN by phone, fax, or mail. There is no
cost and the
process is quick and easy.
